Why the combination is so dangerous
Both drug types act on the same chemical pathway, at different points along it.
Nitrates work by donating nitric oxide, which tells blood vessels to relax and open. ED drugs work by blocking the enzyme that clears away the signal nitric oxide produces. So a nitrate floods the system with the signal, and a PDE5 inhibitor stops it being switched off.
The result is not two mild drops in blood pressure added together. It is a runaway effect — vessels throughout the body dilating with nothing to stop them — and it can produce a collapse severe enough to cause a heart attack, a stroke or death. This is why it is an absolute contraindication rather than a caution.
The medicines that count as nitrates
This is where men come unstuck. A good number of these do not sound like nitrates, and men on them frequently answer 'no' in good faith when an intake form asks.

Why there is no home waiting rule
Do not use a countdown as permission to take a nitrate. If chest pain occurs after an ED medicine, get emergency help and tell the team exactly what you took and when.

The Viagra label says safe nitrate timing after sildenafil is unknown, including at 24 hours. The Cialis label permits considering a nitrate only after at least 48 hours have passed since the last tadalafil dose, and only when medically necessary in a life-threatening situation, with close medical supervision and blood-pressure monitoring. These are emergency clinical decisions, not instructions for taking nitrates yourself.

The emergency scenario, and what to say
Here is the situation that actually kills men, and it is worth reading twice.
You take an ED drug. Later that night you get chest pain. An ambulance is called, and the standard first treatment for suspected cardiac chest pain is nitroglycerin under the tongue — which is exactly the drug that must not be given.
Tell them. Say plainly that you have taken an ED medication and when. Paramedics and emergency doctors deal with this constantly, nobody will be embarrassed, and there are other treatments they can use instead. The only thing that makes it dangerous is not saying so.
Riociguat: the other absolute no
Riociguat, sold as Adempas, is used for pulmonary hypertension. It is not a nitrate but it acts on the same pathway from a different angle, and the combination with an ED drug is contraindicated for the same reason.

Alpha-blockers: a caution, not a ban
Drugs like tamsulosin and doxazosin — taken by a great many men over 50 for prostate symptoms or blood pressure — also lower blood pressure, and combining them with an ED drug can cause dizziness or fainting.
This is a genuinely different category from nitrates. It is manageable rather than forbidden: usually by being stable on the alpha-blocker first, starting the ED drug at a low dose, and separating the two by several hours. It needs a prescriber who knows about both, which is an argument for one who can see your whole medication list.
What this means for buying online
The nitrate question is the reason safety disclosure carries the most weight in every review on this site. A page that sells a prescription vasodilator without naming the one interaction that can kill you has failed at the only part that is not marketing.
Two practical tests when you are assessing any service. Does the page tell you about nitrates before you hand over money, and does it explain the consequence rather than just listing the word? And does the intake form ask specifically about heart medication, by name, rather than a single vague box about 'other conditions'?
