What a legitimate service looks like
- A licensed clinician reviews your case. Either by video, phone, or a written questionnaire reviewed by a real person licensed in your state. There is always a human decision.
- They ask about nitrates and heart disease. This is the safety question that matters most. An intake form that does not screen for nitrate use is not a medical intake form.
- They use a licensed US pharmacy. You should be able to find its name and verify its licence with your state board of pharmacy.
- They can refuse you. A service that approves everybody is not assessing anybody.
- Pricing and cancellation are clear before you pay. Subscription terms in particular should be visible without hunting.
The warning signs
- 'No prescription required.' In the US, all four ED drugs are prescription-only. Any site offering them without one is operating illegally, and everything else about it should be assumed to match.
- No clinician involved at any point — you fill a form, pay, and pills arrive.
- Prices far below everyone else. Generic sildenafil is already cheap. Prices dramatically below the market usually mean the tablets are not what they claim.
- No verifiable physical address or pharmacy name.
- Spam email or text as the first contact. Legitimate clinics do not acquire patients that way.
- Foreign shipping with no US licence. Importing prescription drugs for personal use is generally illegal, and there is no recourse if the product is wrong.
Why counterfeits are genuinely dangerous
ED medication is one of the most counterfeited drug categories in the world, because demand is high and buyers are motivated not to ask questions. Seized counterfeit tablets have been found to contain no active ingredient, several times the labelled dose, or entirely different drugs.
The same problem affects 'natural' ED supplements. The FDA has repeatedly issued warnings about supplements found to contain undeclared sildenafil or tadalafil. If you take nitrates and believe you are swallowing a herbal capsule, that is a potentially fatal surprise.
The real limitation of online care
Even a good telehealth service has a narrow view. It can assess you for a prescription. It cannot take your blood pressure, run a lipid panel, check your HbA1c, or examine your prostate.
Given that ED in a man over 50 is frequently the first visible sign of vascular disease, that gap matters. The sensible pattern is to use online care for convenience and cost while still having an in-person check with a primary care doctor. Why the heart work-up matters.
