Erections depend on healthy blood vessels. The habits that clog your arteries are the same ones that soften your erections, which means the changes that help your heart usually help here too. This is slower than a pill, but it is the only option on this list that makes the underlying problem better.

Why this comes first

An erection is a plumbing event. Blood has to arrive quickly through the two arteries that feed the penis and then stay put. Anything that stiffens or narrows arteries — high blood pressure, high blood sugar, cholesterol plaque, tobacco — reduces both the flow in and the pressure that holds it.

The arteries in the penis are roughly 1 to 2 millimetres across. The arteries feeding your heart are two to three times wider. The same amount of plaque blocks the smaller pipe first, which is why ED so often shows up several years before chest pain does. Treating it as a plumbing warning light rather than a bedroom problem is the single most useful reframe most men get.

What actually moves the needle

Aerobic exercise. The best-studied target is around 40 minutes of moderate activity, four times a week. Brisk walking counts. Studies that hit roughly that dose for six months found real improvement in erectile function, and the men who started out worst improved most.

Losing weight if you are carrying it. Fat tissue converts testosterone into oestrogen and worsens insulin resistance, and both work against erections. A loss of around 10 percent of body weight is where men typically start noticing a difference.

Stopping smoking. Nicotine constricts blood vessels within minutes of use, and long-term smoking damages the vessel lining permanently. Improvement after quitting is common but not guaranteed, and it is better the earlier you stop.

Cutting back on alcohol. Alcohol is a depressant of the nervous system. Heavy regular drinking damages nerves and lowers testosterone; even a single heavy night reliably interferes.

Treating sleep apnoea. Loud snoring plus daytime exhaustion is worth investigating. Untreated apnoea drops your blood oxygen hundreds of times a night and suppresses testosterone. Men who start CPAP often report erections improving as a side benefit.

A Mediterranean-style diet. More vegetables, olive oil, nuts, fish and whole grains; less processed meat and refined starch. This is the eating pattern with the most consistent evidence behind it for erectile function, most likely because of what it does to blood vessel lining.

Pelvic floor exercises

Often dismissed as something only women do. In a controlled trial of men with ED, pelvic floor muscle training restored normal function in about 40 percent of participants and improved it in another 34 percent — results that hold up well against a first-line drug.

The muscle you want is the one you would use to stop yourself urinating midstream. Squeeze it, hold for a slow count of three, release for three. Three sets of eight to ten, twice a day. It takes about three months to know if it is working for you.

How to combine this with everything else

Do not treat this as an either-or. The standard approach is to start a pill so that sex works again now, and to work on the underlying causes at the same time. Some men eventually need a lower dose or stop the pill entirely. Many do not, and that is fine too.

Common questions

How long before I notice anything?

Most studies that show a benefit ran for at least three months, and six months is a fairer test. Give it that long before you decide it has not worked.

Do supplements like L-arginine or horny goat weed help?

The evidence is weak and inconsistent, and supplements are not tested for purity in the way medicines are. The FDA has repeatedly found ED supplements spiked with undeclared prescription drugs — which is dangerous if you take nitrates. Save your money.
